About Functional Medicine and Lifespan Integrative Medicine
DO YOU TAKE INSURANCE?
Because nearly all of what we do at Lifespan is not covered my most insurance plans, we do not bill insurance. At your request we will provide you with a superbill which you may submit to your insurance carrier. Nearly all of our services do qualify for HSA and Flex account spending.
DO YOU HAVE A PAYMENT POLICY?
We ask that payment be made at the time of service for patients seen in the clinic. Patients utilizing virtual appointments are sent an invoice shortly after the visit.
WHAT IF I HAVE TO CANCEL AN APPOINTMENT?
We ask that patients give at least a 24-hour notice to us if an appointment needs to be cancelled or rescheduled. This allows us to fill that time slot with another waiting patient. For cancellations less than 24-hours we charge a $50 fee (with exceptions for illness or family emergency).
Since we prepare your custom IV prior to the patient visit, "no-shows" and same day cancellations are charged half the cost of the IV.
DO YOU OFFER VIRTUAL VISITS?
Yes. New patient consultations and follow up visits can both be scheduled virtually. We offer our visits through a HIPPA compliant video platform.
WHAT ARE YOUR SERVICE FEES?
Our fees vary depending on the type of service. Below are typical prices, but please call to confirm if you have questions.
New patient Functional Medicine consultation (55 min), $350 - Includes reviewing health history, ordering tests, lifestyle changes, and/or IV recommendations if applicable
Sublingual immunotherapy, new patient (30 min), $249 - health intake, lab orders**
Sublingual immunotherapy, follow up visit (15 min), $75 - Required every 3-6 months for refills
IV therapies - Price varies by each IV but ranges from $75-$250, and nutrient injections range from $25-$125
Aesthetic procedures - Please call for pricing
Please note that bloodwork and specialty labs are not included in the consultation fees. As a courtesy to our patients, we offer specialty labs at cost. Furthermore, supplements are offered to our patients at a discount off retail price.
